INTENSIVE CARE COORDINATOR
Location: Athol, Northampton
Working directly with children and families through the state’s Children’s Behavioral Health Initiative, facilitate the development of Care Planning Teams for youth with serious emotional disturbances (SEDs) enrolled in MassHealth. In concert with the Team, the Coordinator develops family-centered Individual Care Plans, and actively assists the child and family to obtain and monitor the delivery of available medical, educational, social, therapeutic, or other services. Bachelor’s or Master’s degree in mental health discipline or equivalent experience required. Strong interpersonal and organizational skills. Must have valid driver’s license and reliable transportation.
