Job Description:
Provide community based employment supports for individuals with developmental disabilities, utilizing a team-based approach. Provide direct supports designed to assist assigned individuals to retain employment and develop independence in work setting.
Qualifications:
High School diploma/GED.
Ability to self-initiate and work independently.
Strong interpersonal and communication skills.
Experience with and commitment to community based supports to individuals with developmental disabilities.
Valid driver’s license.
Able to meet required competency level in Microsoft Office Software.
Physical Requirements:
Extensive travel required.
Frequent movement within community setting including but not limited to bending, sitting, lifting and carrying.
Ability to sit for moderate time periods.
Ability to use computer.
Responsibilities:
Provide direct service support in the workplace as assigned, in response to the needs the individuals.
Provide direct care teaching and modeling to individuals as needed.
Provide support and assistance to individuals necessary for them to attain their personal objectives relate to employment opportunities.
Participate in the planning process to ensure adequate support in the areas of health and safety.
Demonstrate commitment and enthusiasm in supporting individuals in creative and innovative ways.
Assist in the development of the Individual Support Plan.
Provide required assistance with any personal care routines as needed.
Provide support and assistance to individuals necessary for them to develop and maintain relationships with others.
Provide transportation as appropriate in response to individual schedules and desired employment activities.
Work cooperatively/collaboratively with other individuals within the employment setting.
Ensure that individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information.
Ensure completion and maintenance of documentation as required by KDC and all applicable funding agencies.
Participate in staff meetings, group and individual supervision, and case reviews as necessary.
Maintain compliance with all Kennedy-Donovan Center policies and procedures, DMR regulations and requirements and quality service standards.
Perform other job-related responsibilities as assigned.
