Clinical Social Worker-Tewksbury, MA
General Statement of Duties and Responsibilities –– Provide clinical social work services to a high risk caseload of young and middle aged patients, all over 18, admitted for short-term, sub-acute medical stabilization, reconditioning, rehab and discharge back to the community in a timely manner. The patient population includes a heavy concentration of patients with numerous and complex psycho-social components that impact on medical care and treatment as well as discharge planning: poly-substance abuse/addictions; homelessness and poverty; chronic neuro-psychiatric/neuro-degenerative illnesses and marginal/impoverished support systems. The caseload also consists of patients with status-post traumatic brain injuries and acquired brain injuries admitted here from acute rehab hospitals for ongoing rehab care and discharge planning to a community setting. Although the primary responsibilities for this position are case management and discharge planning services, other responsibilities include: patient/family education, crisis intervention, high risk screening, psychosocial assessments, interdisciplinary team planning and documentation in the electronic medical record. This assignment is primarily on one patient care unit with additional responsibilities to provide backup coverage to other units during periods of staff shortages and/or disproportionally high caseloads.
MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE: None. (See Required Education and Licenses.)
REQUIRED EDUCATION: A Master's or higher degree in social work.
LICENSES: Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Class D Motor Vehicle Operator's license or the equivalent from another state.
- Current and valid licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work.
This is a Civil Service position.
37.5 hours per week, days off Sat./Sun.
