Position Title:

Recovery Counselor/Case Manager

Position Description:

Transitional Age Youth (TAY) Outreach Program Mental Health Recovery Services, Franklin County. Full-time Transition Facilitator/Case Manager needed to provide case management and outreach recovery services to young adults, ages 18-25, struggling with mental health issues. Facilitate their transitions to independent living in Franklin County. Hours are second shift Wednesday through Sunday. $11.40 per hour, excellent benefits. Responsibilities: Work with hard to reach young adults; Maintain a caseload of outreach participants; Foster a transition to independence by linking participants to state services and community resources, coordinating participant entitlements; Encourage meaningful activities, such as employment, education etc; Promote recovery from/adaptation to mental illness; Use Strengths based perspective to build a strong commitment to health and wellness. Minimum Requirements: Ability to work independently and with a team; Professional experience working with young adults/adolescents preferred; BA or equivalent experience preferred; Valid driver's license and reliable transportation required; Electronic Medical Record system, MAP, CPR, and First Aid certifications required (agency will train).
