Position Description:

Personal Care Service Provider (part-time) Moore Options for Seniors About the position: The purpose of this position is to provide services as outlined in the client care plan that support our elderly and disabled clients as they continue to live in their homes and communities. This position assists individuals with necessary services that allow them to maintain their highest level of independence and provides them with the support they need to live a high quality life outside of an institution. This current opening is in Greater Manchester. This is a part-time position. Must be available on weekends and some weekdays. Essential skills and responsibilities: •Provide direct services to the individuals we serve as identified in the client Plan of Care. •Successfully complete mandatory staff training and development curriculum and participates in ongoing staff education as directed by supervisor. •Provide personal care/assistance and homemaking services as needed, including but not limited to bathing, dressing, housekeeping, laundry, meal prep, transportation, errands. . •Build/create a positive relationship with the individuals we serve and with his/her family and/or representative. •Provide complete, accurate and timely documentation as outlined in MOS process. •Communicate and document important information to supervisor or director. Notify supervisor or director of any significant changes or issues that may affect the safety, health or well being of the client. •Demonstrate honesty, independence, self-motivation. •Other duties as assigned by supervisor. Qualifications: Basic Qualifications: •High School diploma or equivalent •Experience in the field of Gerontology preferred •Valid NH driver’s license and has access to insured vehicle during working hours as required for role. Skills/Abilities: •Ability to successfully serve older adults with varying abilities exercising consistency and critical thinking. •Moore Center Services considers it in the best interest of the clients we serve that the ability to speak, read and write English are essential functions of all positions. •Maintain a positive attitude including the use of positive language, manner and behavior while working with the individuals we serve and with team members. •Ability to work independently within the guidelines of the client care plan to maintain and improve the clients’ quality of life in the community. •Demonstrate the ability to be an effective team member. •Demonstrate a strong emphasis on customer service skills, both to our clients and all staff. •Ability to successfully complete assignments with considerable autonomy. •Ability to provide basic homemaking services to insure safe, clean environment for the individuals we serve. •Demonstrate flexibility in scheduling and daily assignments. •Demonstrate understanding of personal and professional boundaries related to the provision of personal care services.
