Position Description:

Child and Family Services is an independent nonprofit agency dedicated to advancing the well-being of children by providing an array of social services to strengthen family life and by promoting community commitment to the needs of children has an immediate opening for an ISO Case Manager (focused in the Seacoast area).

An ISO Case Manager is responsible for managing in-home ISO (Individual Service Options) and ISO foster care cases as assigned. The case manager will manage the case from the point of referral to discharge, assuring that the child and family are provided timely, appropriate and effective services. The Case Manager may supervise and support ISO foster homes as well as coordinate other services (i.e. parent aide, family therapist) involved in the case. The Case Manager will work in partnership with the referral source and other stakeholders. The Case Manager is responsible for maintaining communication with, and the satisfaction of, the referral source as well as the development and dissemination of all case related documentation. The Case Manager will be available for day and evening home and community based meetings as convenient for the child, family and foster family. Bachelor's degree with 5 related years experience required, Master's Degree with 2 years preferred. Ideal candidate will be detail oriented, be an efficient time manager with the ability to prioritize work load. Valid driver's license and auto insurance required. This position is statewide position focused in the Seacoast area.
