Position Description:
| Full-time Case Manager to work in neighborhood health clinic's elder service program. Will conduct health and functional assessments of program enrollees, assess for Personal Care Attendant services, assist in developing service plans, determine need for community long-term care services, work in team to monitor status.
Minimum qualifications: Bachelor's degree in Social Work, Human Services, Nursing or related field. Must have valid driver's license and/or reliable transportation and good driving history. Experience in social services/case management required. Requires written and spoken fluency in Spanish and English.
