Bi-Lingual HIV Case Manager

Tapestry Health seeks a dynamic candidate to fill the position of HIV Services Case Manager in our Greenfield office Monday -Friday (8am-12pm). The Case Manager position will provide direct services and administrative support to the agency's HIV Client Services department at Tapestry Health.
Minimum Required Qualifications: Associate's degree or two years of college required. Significant job experience may substitute. Two year’s experience in case management or similar casework; evidence of life experience or knowledge of program's target populations and local service delivery systems. Excellent written and verbal communication skills; able to sit at desk and complete reports, forms, and other data collection tools accurately.
Required: Bi-lingual English/Spanish, Current Driver's License with access to vehicle
Preferred Experience: Education- Bachelor's Degree, Computer literacy in Microsoft Word, Microsoft Access and Excel
Compensation: $16.38 per hour
