Position Description:
| Full-time Adult Family Care Social Worker to provide psychosocial evaluation and assessment of referred individuals and potential host families/care providers. Assists in matching potential participants with host families. Helps develop care plans, goals for participants. Requires good assessment skills, home visits, knowledge of community services.
