Position Description:

(part-time) Bi-Lingual HIV Case Manager Location : Greenfield Company : Tapestry Health Systems Website : http://www.tapestryhealth.org Description : Tapestry Health seeks a dynamic candidate to fill the position of HIV Services Case Manager in our Greenfield office Monday -Friday (8am-12pm). The Case Manager position will provide direct services and administrative support to the agency's HIV Client Services department at Tapestry Health. Minimum Required Qualifications: Associate's degree or two years of college required. Significant job experience may substitute. Two year’s experience in case management or similar casework; evidence of life experience or knowledge of program's target populations and local service delivery systems. Excellent written and verbal communication skills; able to sit at desk and complete reports, forms, and other data collection tools accurately.
