CHD is actively recruiting qualified candidates for its Pilot Community-Based Flexible Supports (C.B.F.S.) Outreach Team. This is a DMH contracted service working with adults with lived experience of being diagnosed with symptoms of mental illnesses, histories of trauma/abuse, and addictions. We are seeking part-time Direct Care Outreach Workers to work as members of a multidisciplinary team developing therapeutic relationships with participants; implementing rehabilitative interventions; providing community support, including transportation; and administering medications through the MAP Program. Additionally, the Direct Care Outreach Worker will implement motivational interviewing techniques and complete documentation in an Electronic Health Record system.
Bachelor’s degree in a related field preferred. Demonstrated experience working within the community mental health system and working with individuals with lived experience of being diagnosed with symptoms of mental illnesses, histories of trauma/abuse, and addictions required. Must be at least 21 years of age and have a valid driver’s license and reliable vehicle. Bilingual/bicultural candidates are encouraged to apply.
