INTENSIVE CARE COORDINATORS
Working directly with children and families, Care Coordinators will facilitate the development of Care Planning Teams for youth with serious emotional disturbances enrolled in MassHealth, developing family-centered Individual Care Plans and actively assisting the child and family to obtain and monitor the delivery of available medical, educational, social, therapeutic, or other services.
Bachelor’s or Master’s degree in Social Work, Psychology, or related discipline and demonstrated child experience required; minimum two years’ experience in community behavioral health setting preferred. Strong interpersonal and organizational skills. Must have driver’s license and reliable transportation. Full-time.
