Centralized Intake Clinician
Location:Newport & St. Johnsbury, VT
Description: Provide computer based centralized intake assessment. This position is primarily office based with some telephone contact with clients, clinicians, and family members. Work is based both in Newport and St. Johnsbury.
Duties: Provide clinical centralized intake assessment (Diagnostic, need and strength base) and coordinate with clinicians within our adult, children and family, substance abuse and developmental services programs.
Collaboration with other agencies, hospitals and other providers inside and outside NKHS.
Coordinate with NKHS emergency services and case management providers.
Coordinate referrals to longer-term service providers like DS or CRT.
Provide consultation to treatment providers after centralized intake referral.
Maintain case records for population served within the centralized intake program.
Qualifications: Requires Master’s degree in psychology, social work, or counseling, and relevant licensing or eligibility for licensing.
Prior experience in a community or inpatient mental health setting in a position inclusive of clinical assessment of adult and children with mental health and/or substance abuse and/or individuals within developmental services would be preferred.
Must have reliable transportation and valid driver's license.
