Position Description:
| Full Time (35 hours) Due to program growth.
Responsible for participating in the initial and on-going assessment of the health functions of enrollees, determines appropriateness for alternatives to institutional long term care services and developing community based care plans. Bilingual Spanish preferred but not required.
REQUIRED: Bachelor degree, two years professional experience in the care of persons over 65, valid driver’s license and reliable transportation. LSW is preferred. This position requires effective communication skills, computer skills and attentiveness to details. Salary in the thirty’s.
