Case Manger - Middletown, CT
The Case Manager provides comprehensive pro-social and strengths-based, case management services for clients in program. Duties include community linkages, fulfillment of basic needs, use of motivational interviewing and reporting.
REQUIREMENTS/EDUCATION AND EXPERIENCE/QUALIFICATIONS
Bachelor’s degree plus 2 years of experience in administering assessments and performing case management functions. Education may be substituted with experience on a year for year basis.
Displays excellent work habits (attendance, promptness, timeliness, professionalism), is open to feedback, and makes good use of time. Proven ability to work on a team to ensure program goals are met, communicates effectively verbally and in writing, and is committed to personal and program growth.
