Community Options Resource Specialist
Full time (35 hrs) for Elder Service Agency in Turners Falls. Work with internal interdisciplinary team, nursing facility & hospital staff. Offer outreach & counseling to consumers & caregivers on community based options, conduct assessments and develop comprehensive community based service plans. Requires: Bachelor’s Degree; strong clinical & assessment skills; excellent communication, time management and computer skills; ability to drive with insured, reliable transportation.
